Revised Medical Form Needed as of January 1, 2012

Many people are currently planning to file adjustment of status (I-485) applications for permanent residence. A note to all adjustment applicants that the I-693 medical form was changed as of January 1, 2012. If a medical examination occurred on or after January 1, 2012, the civil surgeon must use Form I-693 with a revision date of October 11, 2011. The revision date is printed in the lower right margin of each page of the form. Applicants may wish to verify that the correct version of the form is used, to avoid any delay in U.S. Citizenship and Immigration Services (USCIS) processing of their I-485 adjustment cases.
